I love to provide individualized care, so each patient can look and see their best in every aspect of their life. Hobbies/Interests: I love trying new restaurants, seeing live music and theater, and spending time with my husband, son, and French bulldog.

Small phrases and moments like this drive me to give patients the best vision they can have. Optometry allows you to have many special moments with your patients, and I am thankful for the opportunity to share it with them. What does providing a transformational eyecare experience mean to you? I believe transformational eyecare experience comes from truly listening to patient needs and acting on those needs. Hobbies/Interests: My life outside clinic revolves around my dog, dancing, and filmmaking.

Professional Achievements and Associations: Multiple Rotations in the Boston VA and Community Health care systems; Optometrist with Retail settings since 2019; Member of the Maryland Optometry Association. Clinical Certifications and Specialties: Primary Care Optometrist. The reason I became an optometrist is: Growing up, I used to think I wasn’t good at school. When I got my first pair of glasses in 7th grade, I became the top student in my class till college through high school. I made a pledge since then to make a positive impact in people’s life. My favorite part of helping my patients is: Helping patients see better to improve their activities of daily living and diagnosing possible life-threatening conditions in the eyes. In my spare time, I enjoy: Football, soccer, rugby, working out and Cooking.

Professional Achievements/Associations: Diplomate of the American Board of Optometry, Member of the American Optometric Association, Michigan Optometric Association, and West Michigan Optometric Association. Clinical Certifications & Specialties: Practicing primary care optometry including pediatrics, fitting glasses, and contact lenses. Specializing in diagnosing and managing ocular diseases including Dry Eye, Cataracts, Glaucoma, Macular Degeneration and Diabetic eye disease. How are you helping patients live their best lives? By providing the best care and individualized treatment plan tailored to the specific needs of my patients. How are you impacting your local community? I serve on the board of trustees for the Association for the Blind and Visually Impaired (ABVI). I belong to the Grand Rapids Lions Club supporting the GR Lions Club Vision Clinic at Cherry Health, ABVI, and local service projects.

Dr. Gerald Eisenstatt is a graduate of Southern College of Optometry, and has been in practice for 35 years. Dr. Eisenstatt is a member of the American Optometric Association, the Tennessee Association of Optometric Physicians, and past president of the West Tennessee Association of Optometric Physicians. Dr. Eisenstatt retired as an Associate Professor of Practice Management at Southern College of Optometry where he received the distinguished Teacher of the Year award from the senior class for several years. He was also the Director of the Hayes Center for Practice Excellence, and a member of the Practice Management Educators Association. Dr. Eisenstatt received his M.B.A. from Christian Brothers University in 2011. He is married to Lynn and they have two sons, Brian and Michael.

The reason I became an optometrist is because I have a strong interest in the anatomy of the eye and strong passion for making quick impact in someone life. My favorite part of helping my patients is the satisfying feeling of making a quick impact in someone vision and the satisfying feeling of being trusted with someone vision. How are you helping patients live their best lives? I approach each patient with precision and care, guaranteeing excellent patient outcomes. The best interest of each patient is essential in my care plan, and I always strive to understand what the patient is seeking to improve their quality of life. In my spare time, I like to travel, paint, cook, and read.

The reason I became an optometrist is: I love to help people - and what better way to do it than by giving the gift of sight and preserving their eye health. My favorite part of helping my patients is: I love all of it, but making those lifelong personal connections is at the top of the list! In my spare time, I enjoy: Baseball; Reading; Beach; Time with family and friends.

How are you impacting your local community or helping your patients live their best lives? I love being able to help our patients see by not only providing glasses but also by managing ocular diseases. Being able to help provide comprehensive eye exams is why I love doing what I do! Hobbies/Interests: Reading, watching scary movies, watching IU basketball, listening to music, and spending time with my husband and family.

Undergraduate: Miami University (Oxford, OH). Optometry School: Indiana University School of Optometry. How are you impacting your local community or helping your patients live their best lives? I like to do my part by making sure every patient leaves our office seeing as clearly as possible, has all their eye health needs met, and has each of their questions answered to the best of my ability. I provide detailed, comprehensive eye exams for adults and children, diabetic eye exams, contact lens evaluations, and urgent eye health visits. Hobbies/Interests: When I’m not seeing patients, I love to spend time with my husband and two kids, check out a local park/playground, or splash at the neighborhood pool. I enjoy working in our garden, reading, listening to podcasts, and hitting the gym.

How are you impacting your local community or helping your patients live their best lives? By actively improving my patients’ vision. I provide comprehensive eye exams for adults and children, diabetic eye exams, contact lens fitting, binocular visual dysfunctions, and ocular disease management. Whether it is resolving a problem or improving our sight with new glasses and contacts, I believe that when our vision improves, so does our lives! Hobbies/Interests: Rooting on the Bucs down at the Ballpark and the Steelers down in my Living Room; Sports like Dek Hockey, Spikeball, and Pickleball; Chess and some other Trading Card Games.

Dr. Jacqueline Escobar is an optometrist devoted to helping patients achieve optimal vision and eye health in a comfortable and caring environment. She has a Doctor of Optometry degree from Midwestern University - Chicago College of Optometry, where she gained the knowledge necessary to provide patients with comprehensive eye examinations and the tools to properly diagnose and treat a wide range of eye conditions. During her personal time, she enjoys reading, traveling, nature trails, and spending time with family, friends, and her sweet pup.

For example, I have loved working with the See to Succeed program, which offers free comprehensive eye exams and eyewear to underprivileged students in Houston. Hobbies/Interests: Outside of work, I enjoy traveling, trying new foods, reading, hiking, and spending quality time with my husband, family, friends, and 3 pets.

Providing a transformational eyecare experience means addressing all the patient’s needs, visually and medically, and have them leave knowing their best interests were addressed during their visit. Dr. Evans graduated Magna Cum Laude from Houghton College with a B.S. in Psychology and Writing. She was accepted to a Master's Program at UCONN but decided to switch fields. She obtained her Doctorate of Optometry degree from S.U.N.Y College of Optometry in 1995 with externships at Westpoint Military Academy and Castle Point V.A. Hospital. She was awarded the Frederick W. Brock Memorial Award for Outstanding Clinical Performance in Vision Therapy and the therapeutic Pharmaceutical Agents Advanced Practice Certificate in 2002.
